chronic pain and pulse and blood pressure

I'm looking answers for my chronic pain,pulse and blood pressure keeps going up high and the next minute very low. Do I need a new pacemaker ? And another question is why did they put this in ? My pulse was at 15 and dropping and no medication would keep it normal. They say I did have a heart attack. Now I have artrist in my rib cage and trigcardia.
