Dear Friends,
I have had pacemaker for more than 3 years now, but lately I have started noticing that when I get into high pitch while talking my head starts getting heated and I start feeling light headed, anyone else have ever experienced this or knows what it could be?
thanks

I would certainly advise you get some general checks as I previously mentioned and perhaps pacemaker checks with your clinic if this is a new, sudden symptom. Do you have home monitoring? You could always try to send a transmission to your clinic for them to have a look at the data. They might be able to see something from your downloads.
I would have a word and describe your symptoms to both your general doctor and pacemaker clinic. I presume you have annual pacemaker/battery/lead checks? My pacemaker is three years old too so it shouldn't be a battery/lead problem you are getting, but they may want to make some checks.
Try not to worry but do seek advice. Good luck
What happens when you constrict your airway?
by Selwyn - 2021-03-19 16:07:49
High pitched singing and speaking is only possible when the vocal cords are tightened. This constricts the airway. The result is an increase in intrathoracic (chest) pressure. The increased pressure within the chest decreases venous return ( ie. blood going back to the heart). The result is :
1. Venous congestion of the veins above the heart ( =heat)
2. Slowing of the heart ( as venous return is reduced)
3. A subsequent DROP in blood pressure. (2+3 = light headedness)
Having a pacemaker worsens these symptoms as the PM is not able to respond quickly to the change in circulation.
See Valsava's Manoeuvre for further information.
I wonder whether you resting pulse rate needs to have a little increase?
Head gets heated and feel lightheaded

Probably when you become excited, your blood pressure goes up. When this happen, some people fell light headed. Of course, you will need to corroborate it by taking your blood pressure at that precise moment. I don’t believe that your experience has anything to do with your pacemaker.

I would speak to your general doctor

Hello Dalbirsingh87,
Have you checked your blood pressure recently or do you have any arrhythmias? When you say it occurs when you get into high pitch when you are talking, do you mean like when you raise your voice due to anger, since stress of any kind can raise blood pressure and this can cause lightheadness.
I would have a word with your general doctor and get some blood pressure, arrhythmia and blood checks as soon as possible.
My arrhythmia (AFib) was first detected with symptoms not too dissimilar. I got sudden head bursts of pressure/heat as though I was having a transient ischaemic attack. I did in fact loose consciousness momentarily but I was seated at the time so no harm came of it. But it was frightening. Do you have a history of arrhythmias?
I hope for the best
