I had my CRT-D implanted March 2016, while for the first 4-6 months it was more sore and painful than I bargained for, it had settled down.
Just about 10 days ago I went for an ultrasound and as part of the examination I needed to hold my arms above my head for 10 mins or so. 2-3 days after I started to feel a little tender where the leads dive down beneath my collar bone. It continues to be sore.
I guess I've just pulled the tissues where my leads are attached, but wondered if anyone else has had a similar experience? It's just a bit achy again and I thought that was all behind me.

In fact after the final "tweaks" of my pacemaker programming at the one year check up it is working so well that I forget I have it.

It's certainly possible that you've been favoring this arm since your impant and it's stiff, with scar tissue keeping the soft (and hard) tissue from flexing completely. My guess is that it's a more minor variation of "frozen shoulder". The solution is to keep doing it. Use it or lose it.
Remember, pain is nature's way of telling you that you're still alive. ;-)
