Pacemaker for type two block

Medicare does not allow a pacemaker for type two heart blocks ,  Say I bought the pacemaker myself. Almost all cardiologists depend on a medicare practice and will lose their ability to have medicare if they proceed with such a pacemaker, purchased by the patient.

 

How can one get around this problem?
