The services I speak of are for my own machine. I'm trying to reduce cost not add by adding Medicare plan.
NOAC

You didn't say why you were taking Warfarin. I assumed because of an arrhythmia but if it's for a valve problem, forget the NOAC suggestion.
NOAC == Novel Oral Anti-Coagulants. That is, Eliquis, Pradaxa, and Xarelto. Of the group, Eliquis is the safest but the others have their uses, too.

I am just grateful to God that I lived long enough to have my ICD put in. So many people are not as lucky as us; even though we sometimes don't feel lucky.
INR cost

I assume you're talking about the USA. All medical costs are all over the board. You'll have to find out what it costs in your area. Even then, I wouldn't be surprised if ithe cost were different every time you use the lab. I've never been able to figure out costs.
Two suggestions. One, but a meter and do it yourself. This isn't an outrageous proposition anymore. Second, since you know you'll have these expenses, buy a Medicare supplemental plan that includes lad testing. ...and a third option would be switch to a NOAC. No miss, no fuss, and for the vast majority of us, much safer.
