I use a hot tub quite often, and have had no problems, though obviously I agree with Theknotguy and your wound needs to be fully healed first, and other meds may be a factor - I'm on quite a few and no issues.
If you do start to feel odd, get out.

I'd discuss with your EP first. If he/she says no, it's a no.
It won't bother the pacemaker but you have to be careful because of the heat and what it might do because of your heart issues. You also don't say how long you've had the pacemaker and if the wound has healed or what. Obviously you don't want to get into hot water with a wound that hasn't healed.
Depending upon your heart problems and medication you are taking the heat from the spa could exacerbate your problems. So you want to be really careful.
I ended up working with a licensed massage therapist. She went back for extra training to make sure she wouldn't bother the pacemaker or leads. Been working with her for four years now and she's done a great job. They did CPR on me and broke all the ribs on the right side. So while the pacemaker area is completely healed, the ribs that were broken are still giving me problems after four years.
