Just had my latest checkup at the Cleveland Clinic. Looks like I will be staying on a blood thinner. I'm on Xeralto. Since I have a PM and use a BT, I asked my EP about wearing a medical alert bracelet. He said he recommends it, particularly since I now on a BT. If I were not able to respond to emergency personnel, they may look into my wallet and find my PM card. He said most people don't put anything in their wallets about using a BT. In a critical situation, that could be fatal, so a bracelet is recommended.
He said the good news is that they now have a reversal agent for Xeralto and Eliquis, that acts almost immediately. If they see your bracelet and the BT info, that could save your life.
Since we are a family, I thought you all should know.
The reversal agent is available in the US. I don't know where else it may be available.

Xa inhibitors - blood thinners like Xeralto and their reversal.

I am not sure I would be so optimistic about reversing the effect of Xa inhibitors such as Xeralto. I believe the data is still incomplete and there is a reasonable risk of fatal haemorrhage.

I continue with warfarin, where there is an effective antidote, and an evidence base of efficacy with my medical conditions.
The initial notification that may have jumped the gun a little was:
Connolly SJ, Milling TJ Jr, Eikelboom JW, et al. Andexanet Alfa for Acute Major Bleeding Associated with Factor Xa Inhibitors. New England Journal of Medicine. 2016;375(12):1131-1141. doi: 10.1056/nejmoa1607887
You will note the wide ranging results ( confidence interval). Why was this? There is a substantial follow up thrombosis rate.(18% at 30 days)

According to my doc at the Cleveland Clinic, they have been approved by the FDA and should be used in an emergency. There is danger just being on blood thinners.

I recently discussed blood thinner choices with my nurse at the EP's office, as I am trying to get off meds. She said Predaxa (? it think that was it) had a reversal drug, but it's 3x more expensive the Xeralto. I take Xeralto and with arrhythmias and an ICD and he won't take me off of it. They tell me warfarin is not a choice for me. They are able to give me samples of Xeralto, but never have enouogh of Predaxa.
That would be great if there was a reversal drug for Xeralto. I worry about it all the time being very active.
