I have now had a CRT system for one year. I have basic pacing through the bundle of His.I also have a left ventricular lead. My Ekg went from a typical left bundle branch block pattern to a QRS nearly the same as before the LBBB developed.I do have left axis deviation which the EP doc and I agree ( I am a pulmonary physician who has had a crash review and course in EP 101 over the last year) is likely a left anterior fasicular block.The left ventricular lead is in place "as a precaution " in case the His Bundle pacing for some reason fails and then I would revert to a regular biventricular pacing arrangement.
Question -anyone out there with a His Bundle pacing situation?

I had the PM place one year ago in Houston. I had developed an left bundle block and exercise induced 2:1 AV block. The AV block could have be treated by biventricular pacing with a lead in the right ventricle at the apex.However there is some likehhod of that type of pacing actually causing a deterioration in heart function and even. In my case the presence of the LBBB lead the doctor to go with Bventricular pacing plan and if possible to place a HIs Bundle lead.(Sometime that cant be done for technical reasons) One of the EP docs from Medtronic on their website says that CRT (and HIs bundle pacing ) is basically treatment for LBBB and it is mainly those patients with heart failure and LBBB who experience the most benefits from CRT.
In my case by 2 months post procedure I seemed to be able to run about as well as before the LBBB and the AV block developed and that level of exercise tolerance seems so far to have persisted.

That is interesting. While I have a RBBB, HIS bundle pacing can also be used for right-ventricle pacing as well. It can be used for both. As a matter of fact, it is the most beneficial for RBBB (as I've read), however it is not performed that often. I spoke to a doc at the Cleveland Clinic and asked them about doing HIS bundle pacing for RBBB. They did not want to talk about it. They use a CRT with 3 leads for CHF. I asked, "why won't you do HIS pacing and maybe prevent CHF"? The doc said they prefer using a conventional 3 lead device when it is necessary to pace the left ventricle. Can you believe it? That is.........for now. We'll see what the future brings! Sounds like the docs in Houston are ahead of the game. Great news for you!

...as suggested by the MOST Trial. (Googal His-pacing.org). The business model of some hospitals points to 3 lead CRT because it pays so much, and His pacing only pays the hospital the same as conventional pacing.

If I remember right the Most trial showed that dual chamber pacing was associated with less development of a fib versus V-V pacing.His bundle pacing was not studied in that trial.

Followup to earlier posting about His bundle pacing. In the interim it seems that His bundle pacing has received much attention in the EP journals. More Ep docs are using it for AVblock and also for CRT rather than Bi V pacing. My PM has been working well for the last 2 years as of this month. Any one else out there with a His Bundle pacing PM?

You didn't complete your bio so I am wondering where you are located. I believe that HIS Bundle Pacing is the best possible solution especially for you, but really for everyone. I am curious where you are, because I am interested (and it may be helpful for everyone) to know where it is being performed.
From everything I have read it seems that HIS pacing should provide you with the best longevity. I have read that it can actually reverse CHF, but there are multiple benefits. So I am interested in reading more.
