Where can I find my model and serial number for my pacemaker.?

Hello all, I am new to the forum.  I just had my pacemaker installed a week ago.  In my dicharge papers they were suppose to give me my tempoary ID card.  this did not happen.  How can I find outI the model number and serial number of my unit? I tried calling the hospital, and they plead the 5th amendment.  I called the doctor's office and they said call the hospital or the manufaturer. I can fill out a temp ID card but I don't have all the information I need.  This is a little flustrating. Any Idea's?

You will receive one in the mail in a few weeks.  In the meantime, if you need one, you can call the device company and they should be able to help with a temporary.

Thank you.  I will be doing some traveling next week.  With or without the card.  I thought it would be prudent to have the information I needed.  Some of the areas will have security check points.  I guess I'll find out if they will be understanding or not.  I'll do another post if I run into any problems.  Thank you again.

In 20+  years, I've needed my id card exactly twice, both times at federal buildings in DC. I keep it handy at the airport and any place there is security but I've never needed it. I don't think I've ever needed the make/model/serial number. If you call the manufacturer, they have all of that readily available. At least I assume they all do, I called St Judes once to ask about scuba diving and once I gave them my name, they had my whole history in front of them. So, if something happens that you'd need it, it's just a phone call. If you were to end up in the ER for any reason, they'd immediately contact both your doctor and the device manufacturer so while the card is nice to have, it's really not necessary with today's modern technology.

Did you get a packet of papers at the hospital with the warranty information? Your numbers will be in that, also. Also another paper I've never needed, but I have a nice collection of them now! (I'm on #5)
