Pacemaker Card

Hello. I had a Guidant Pacemaker insert in 2006. I was sent a Medical Device ID card, which listed their contact website as "www.guidant.com."

Anyway, my card is so old that I can't read any of the information on the back of the card, such as model/serial number. I want to contact them to ask for a replacement card. However, their website does not show up. I think Boston Scientific owns them or perhaps owned them.

My question: Does anyone know how to get a replacement card? Not sure if it matters, but it says the leads are Medtronic Leads (but can't read these either).

Yes, BS now owns Guidant.

But they might not have a record of the leads.

And at 10 years it should be nearing replacement time.

So while the PM will be replaced the same leads will be used.

The best bet is to contact the hospital where it was installed and get a full copy of your medical records.

It will probably help when you need to get the replacement.

Here's the 800# for BS patients in USA. 866-484-3268.

They can replace your card. Don't worry about the leads being Medtronic, it will be listed on the card with the model #, etc.

Agree, don't worry about the leads or time for replacement. My leads are more than 15 years old and I am on my third pacemaker. Your Cardiologist who follows you can also help you in the correct direction for a new card with current Physician information. The FDA also required manufacturers to track implanted devices intended to be in use more than 1 year.
