Being a new recipient of the PM, I struggled with the lead decision. I finally settled on one simply because I did not know enough. The Dr. Said he,would choose two, but only when the heart was in rhythm
And mine was not therefore the one lead and also I did not agree to shocking paddles.

Many Different Reasons for a PM

And that determine whether a single chamber or dual chamber PM is used. Also some allowance for predicted future changes and meds.
For example a common reason for a PM is sick sinus syndrome where the primary heart PM, the sinus node, is wacky (to use a technical term). With this it can be too slow, pause, and/or too fast.
In the simplest cases all that is needed to monitor the atria and if the beat is too slow to trigger the atria. But if part of t he problem is that it is too fast that meds are some time used. In that case it might also delay the natural trigger from the atria to the ventricles. In that case a 2 chamber PM work best. It will monitor and if needed trigger the atria and then monitor to see if the ventricle is triggered and if not trigger it.
In other cases the atria is in uncontrolled AFIB and to treat it the connection to the ventricle is removed. In that case the PM is only needed to trigger the ventricles.
