If you can't find an EP willing to do it, ask to have a plastic surgeon assist. This is how mine was done. It might be out of the ordinary for an EP, but it'll be the easiest job the plastic surgeon does all day, and they'll probably do a better job. My EP was thrilled to have the other surgeon step in, meant he could focus on what he does best. Everyone was happy.
