Getting a copy of your report

Hey Everyone,
I sent my 2nd remote transmission a few days ago and yesterday I received a letter saying everything was normal and when my next appointment was, that's it.
I called my Dr.'s office to request a copy of my actual report and they told me I had to go through the "Medical Record Release Dept". I called them and they told me I had to fax a form to them stating I wanted a copy of my report. I asked if they could please put a note on my file to just send them automatically every time and they told me no. They said my signature on the form would be good for a year but I still had to call them each time because they had no idea when my remote transmissions were scheduled.
I understand all the privacy issues but this seems like a bit much to go through each time just to get a copy of my OWN report. I'm sure if anyone else did see it wouldn't make any sense to them and they wouldn't understand it anyway.
I just wanted to share this experience as I know there are some here who have trouble getting a copy of their report so if that is the case ask to talk to the Medical Record Release Department at your clinic or hospital.
Take care and have a good day PM friends!

My Chart
by bluebowtye - 2014-09-27 10:09:17
Hi Lurch,
My clinic (not my insurance company) has that too (called My Chart) but they put very little information on there. About the only thing I have access to is my blood results and my upcoming appointments. They do not put my PM interrogation reports on there even though I requested them to do so. I can send messages to my doctors and make appointments and I think get prescription info but I am not on any meds so I don't know for sure. It shows that I first came to the doctor for syncope and then my heart block diagnosis and that they implanted the PM but it does not go into any detail. It shows nothing about me seeing my primary care physician who is the one who referred me to the cardiologist. I had my annual mammogram and gyno check about a month ago and there is nothing about any of those results, not even that everything was normal. It sounds like yours gives a lot more info and maybe in time mine will too.
Sparrow,
Those are some very good tips. Thank you. When I had my one month check with the PM nurse at my doctor's office I asked for my report and she did give it to me although it was the short version because at the time I didn't know any different. I requested my hospital reports through the medical records release dept and they did give me all the doctor's notes, etc. and the full version of both my interrogation report they did the next morning after my surgery and the one at my one one month check.
I guess it just goes to show we have to be really persistent in obtaining our own health records that we are entitled to.

I have mychart too but again very little information. I wish they would post more but they don't. and it's really hard to get any copies of the reports. the first time I asked I actually just got a letter written for my GP that I could take to him ha. but the next times I've asked nothing. I'm under the impression that they really don't want you to see the records for some reason probably because they think you wouldn't understand it anyway and really have no business getting them?? who knows. oh well.

New Technology

My insurance company just started something new. I can log onto a secure (supposedly) site that allows me to review my medical records.
So far I have found all of my recent (within the past year) reports on blood work etc along with the doctor's write up on my visits (stats, meds, recommendations, referrals).
Have not had an interrogation since they started this new system so I am interested to see if it will show up.
The system is convenient in that I can send a message to my Primary Care Physcian and Cardiologists' office, request prescriptions and make appointments. We will see where this all goes....
