Did someone had similar experience? Coincidentally or not after i got the pacemaker my INR start dropping more and more although my doses gets increased all the time, i drink them 3 years and so far i had stable INR untill i got pacemaker

I've been on Coumadin for 15 years (since my early 20s!) and am still getting tested every week or 10 days with adjustments nearly every week.
For me, everything affects the INR: changes in altitude, changes in sleep habits, changes in exercise regime. It's annoying as all get out! But, at least I haven't lost half my brain to a stroke yet... :)

Wild INR's

All sorts of things can make an INR go berserk.
Mine was stable as a rock between 2.0 & 3.0 for many years - with my PM. Then one day roughly 1 1/2 yrs ago, it started going all over the place. I never changed my Coumadin taking habits. BUT - on Christmas Day it was measured at 7.4. We successfully brought it down, but who knows how long it took to get to that level. Ever since then, it has had no correlation between Coumadin dose & INR.
I was taking 3.5 mg per day when it went high. We cut it back to 2.0 mg/day & my INR dropped below 2.0. Right now I am taking 2.0 mg per day & testing every two weeks. Two weeks ago, it was up to 2.6 on that same dose.
Lack of consistency is one of the main complaints about Coumadin. Looks like I'm suffering that now after years of good results.
