Depends a lot on where you live and who is paying. Sounds crazy?! It is. In the US, the cost varies by locale ie, west coast versus east coast, south versus north, etc. Even a next state can have a wide difference. I have seen quoted anywhere from 20,000 USD to 120,000 USD and no one seems to know why this is. This is only one reason why we need health care reform, but I doubt if we have it any time soon. The so-called Obamacare law doesn't address this at all.
Mine, done in Arizona and partially paid by a "medicare advantage" plan cost just over 35,000 USD of which I had to pay 3300, plus of course the cost of getting there (about 250 miles each way) Makes no sense does it. Oh well, I have it now with only twice annual checkups which are only 50 miles one way travel. Guess I'm just lucky or unlucky depending on how you look at it and what you compare it with.

My pacemaker cost 12,000 but I was lucky enough to have good insurance and didn't have to pay anything!

I'm looking at my Medicare statement for my replacement in 2010.

$9280 for the pacemaker
$9534 for installing it.
$2119 for removing the old one.

The last is the interesting one. $2119 for unscrewing 2 screws and removing the leads. I would have done that myself if I had known. I was wide awake.

So, my advice is to go about your daily routine and forget that you have a pacemaker implanted in your body.
