Hi guys. Had my 4 wk follow up for my new pm. I asked the tech to turn the accelerometer to "on" instead of "on passive". ( I had been noticing some shortness of breath and suspect this setting was to blame. ) Tech sorta raised an eyebrow (thank you PacerRep and Don for the prep) but did turn it on. My first pm was set to "on" as well and I felt pretty good. Plus I asked tech if my AVD was set too short (ha ha ... Another eyebrow raise.. Thank you PacerRep). But tech believed it was set appropriately. I'm still a bit sob today. I know 3 months of inactivity can have a deconditioning effect however I know this symptom and believe its cardiac related. I will test drive my new pm awhile to see how I feel. Just wish I didn't have the sob. It's a bit discouraging.

Hey.. Here's the info. On my device report from June my Percent Paced said : A paced 65% V paced 5% . After new implant in August while in hospital my Percent Paced said: Atrial paced 95% Ventricular paced 99%. While at pm device check Friday I asked the tech why my V pacing was so high. Was my AVD too short? He looked at all my data and said that actually my V is getting paced less than 1%. My new report that day said: Percent Paced Atrial paced 65% Ventricular paced <1%.
I confirmed with my doc that I wanted to be certain I am not getting V paced any more than necessary and he agreed, looked over my numbers and said it looks like it is not.
I have no explanation for the 99%. it was during my hospital stay.
The mode is DDDR. Rythmiq is on. AAIR with VVI backup. This is the same mode on all my reports. The only changes made Friday were turning the accelerometer to "on" . (My activity threshold is med. reaction time is 30 sec. Recovery time 2 min. Response factor is 8. These are same settings as before).
Fallback mode DDIR.
Minute ventilation sensor is on with response factor 3. This is same as before.
The Paced AV delay is also the same on both June pm and August pm at 180msec. Sensed AV delay 150 msec.
Thanks for helping me understand just a fraction of all this. It really helps. I am what you would describe as a thinker and want to know the "whys" and "how's" especially with regards to my heart. Sometimes you just don't get all your questions fully answered when at an appointment. It's awesome to have you in our corner.

Probably just hospital settings

Some of us...myself included.... leave the nominal out of the box settings on overnight and adjust it in the morning before you go home at the pre discharge check.
The reason behind this is, once we start tinkering with the device and activating all the bells and whistles....it can drive telemetry crazy...There's nothing worse than getting called at 3am because somebody in Tele doesn't understand paced rhythms. So we leave all that crap off. Your AVD was definitely too short in the hospital..hence the pacing...then somebody fixed it...It's the only explanation possible.
Don't worry about it, if your under 1% now your set correctly.

What was the tech's reasoning for the AVD being appropriate? Are you in a mode thats called AAI/DDD? or MVP?
