why some heart beats hurt?

pacemaker implant 2007 ..now am all ready use to it but on the night i can feel a heart beat and like the heart massage or moving and the heart beat hurts slow like someone punch me. can that be the pacemaker taking over? i have arrythimias take metropol and my heart is a little thick.
