Advice for taking Entresto

Okay.... Ta..da... drum roll please...

I got my Entresto today and took half of the lowest dose to start; making it easier on my sensitive system. My question is, I'm supposed to take it twice a day, but I already take my Carvedilol twice daily at 7:30am and 7:30pm but I read somewhere that taking the two drugs at the same time was difficult for some... so, how should I take it? My usual method is I get up, have a piece of toast and a few ounces of milk to take my Carvedilol. Then a couple hours later, I eat breakfast and take my other meds and supplements. This seems to work well. Since I'm being taken off Losartin (replaced with Entresto), I can take the first dose with breakfast, but the second dose gets tricky if I need to take it at a separate time from my Carvedilol. Maybe breakfast and dinner? 

Also... is it normal to feel a lot more arythmias and ectopic beats when adjusting to Entresto? Could be the anxiety of it, Lord knows I get anxious over everything these days.

I saw this too: " is it normal to feel a lot more arythmias and ectopic beats when adjusting to Entresto " IMO with any medication changes such as these, it could cause some arrithmias, but you want to make sure that they subside as they WILL have a negative effect on optimizing your CRT. You want that bivp% as high as possible. I alluded to that in another one of your posts but I think you missed it. Hoping your ectopics calm down and I think they will when you get the dosing right.. at least they did for me. good luck :)

We should ideally take our meds according to the instructions on the packet, as prescribed by our doctors.  I see it is usually taken in the morning and in the evening, with or without food, roughly 12 hours apart so that the med is in your system at all times.  Does the med have a dividing line?  Some meds should not be split but swallowed whole especially if they are time release meds when the med is slowly released into the blood stream, rather than hitting your blood stream too quickly and causing more in the way of side effects.  Try not to expect side effects, but do read the leaflet on the possible ones to watch for and when to seek help.

If you get extra beats or other arrhythmias, don’t forget to ask for electrolyte checks since Entresto can cause raised potassium levels.  For this reason you should also avoid potassium supplements or high potassium foods until you get the dosage optimised for you and you know what amounts of Entresto are safe to take.  Your bloods need watching for kidney function too when taking Entresto.

Taking medication is likely to be trial and error until you find the right dose for you.  As USMC-Pacer says, medication such as Entresto may cause an irregular heart beat and affect your heart rate and blood pressure, which could have a detrimental effect on your well being and on CRT effectiveness.  If your arrhythmias do not stop, you may need to reconsider whether Entresto is right for you or whether the dosage needs adjusting?   If you can tolerate the rhythm disturbances for a little while, do so.  Hopefully they will settle and you will start to feel better, but listen to your body Janene and get help if you need to.  Good luck with Entresto

I think you can take both at the same time, providing that the dosis Entresto is low. Otherwise your blood pressure might become too low.

Apart from this, it is better to take these pills late in the evening. Most heart problems arise in the morning and your morning medication is not working (yet). This according to a large and long-term Spanish research project that ran from 2008 to 2018. Why not shift the evening medication to 11 PM?

Janene, first and foremost we are talking here about Entresto being used for your heart failure not for high blood pressure. It is therefore important to take it according to instructions to keep the med in the blood stream evenly over 24 hours.  Taking it once a day at night may not work so well or give full protection.

Having said the above, I agree wholeheartedly with Penguin and H van Dyk that general blood pressure meds can be more effective taken at night since blood pressure starts to rise during the early morning and the majority of strokes/heart events due to high blood pressure can occur during the night too.  Doctors have told me to take my once a day anticoagulant at night too, to better protect from an AF stroke for this very reason

Generally on the prescription bottle it is written when to take and how much at a time.  When in doubt contact  your  Doctors nurse.

Taking Carvediilol and Entresto

The time of onset of valsartan is about 2-4 hours, and the onset of carvedilol is 1-2 hours, so there shouldn't be an issue taking them at the same time. Carvedilol is reccomended to take with food to slow absorption and minimize postural hypotension (low BP when standing up too fast). 

The most important thing to to take them consistently, especially when they are more than once a day. 

BP meds have various onset of effectiveness and peak effect. Some are immediate, some delayed. Your pharmacist can be a wonderful source of advice.

I take my Cardiac Cocktail at 0700 hrs and 1900 hrs.  

Entresto has really been beneficial for me.  My Coreg is back to the minimal dose, due to my low B.P. 

My new Cardiologist tells me my lack of tolerance to the Cardiac drugs means my heart is in poor shape, time to plan for my demise.  Interest because my B.P. has always been low, my tolerance to drugs has always been scary (I never know how my body will react to a drug).  No recreational drugs for me because I do not react appropriately.  

Entresto has really been helpful for me, I am glad I switched over.
