I'm sorry to read this. You're still having pain after three years! This is a very long journey for you. Have you contacted your doctor?  Is there a chance that your pacemaker migrated up further pressing on the collarbone?

 I'm thinking you might benefit from an xray to compare present placement to initial placement. 
 

I am assuming you have no redness or swelling, oozing around the surgical site?

Presumably you've had some contact with your clinicians in a three year period?

Despite the move in the UK to shut down face to face clinics for annual checks in favour of home monitors and "virtual" device clinics, there should have been an opportunity to talk to someone.

Any pain lasting that long needs investigating for peace of mind.
