program changed accidently?

Hi ,
Recently had an operation which required the PM CRTD to be turned off. On completion of the op it was turned back on.
The following day I thought something was different and eventually the PM was checked and found to have a different program. DDIR to DDR I think. How can this happen? Did it go to a default setting?
Thoughts please.

Sloppiness ..playing with some's life ! This is no place for errors ..and I too have made mistakes .. .why ? Naming a program DDIR and another DDR is asking for something to go wrong ...better terminology should e employed ..

They changed the mode for the surgery but didn't put it back to the same setting after.
