Just because you can, doesn't man you should. I have my PM and lead replaced 8 weeks ago and with the exception of a noontime lunch meting or two, I stayed at home for a couple of weeks. The ability to lay down whenever I wanted was invaluable. Take a week if you can. I'm 46 and active
I did

I had my ICD implantation on a Friday night, got out of the hospital Saturday afternoon, and went back to work Monday. I only did half days for that week, though.
For me, it was a severe alteration to my life (self image issues, sudden medical issues I had never delt with before), so I needed to return to at least some normality in my life.
I would probably suggest if you want to go back to work, just make sure that if you start feeling worse, don't feel bad about just going home.

PM or leads?

I would say wait a bit if you can before going back to work, no matter how young/strong. You have gone through some trauma with your body and it needs time to heal and rest.
If you just had your PM replaced and they used your old leads you might not have some of the restrictions... not sure... with using your arm. If you haad leads replaced I would be extra cautious.
If you are at home, try "working" (at whatever you do) at the same pace and stress that your job has for you and see how tired you feel. If one pushes the envelope too hard sometimes we pay a much higher price when something goes wrong. If you are still having headaches and are sore, I would definitely listen to your body and rest if you can.
Good luck.
