What does it mean when my pacemaker tingles? It does it most when I'm upset or stressed out?

Mostly nothing

You haven't said how long you've had the PM, when implanted, where it tingles, etc. So it's like the old joke of,"Doctor, it hurts when I do this." Answer: "Don't do it." Anyway....

Depending upon how long you've had the PM, where implanted, what you're doing, etc. - you may get thumping, poking, tingles, itches, and all sorts of sensations. Unless you're running a fever, or the pain is pretty severe, you can ignore it. It's just your body adjusting to the new reality.

I'm one of those lucky/unlucky people with extra nerve endings in my arteries and veins. So right after I got my PM I was able to feel the PM kicking off my heart. Funny sensation to say the least. I was getting a thumping in my jaw from the better heart beats, pain in my back from the PM pocket, all sorts of stuff. Fortunately as my body adjusted the sensations went away.

Been talking with people with stents in my rehab. They get poking sensations and attribute what they feel to the body adjusting to the stents.

They had the voltage turned up on my PM for the first 90 days so I got a harder thump with each heartbeat. I was having trouble sleeping at night because of the thumping. After 90 days they dropped the voltage on my PM. Less thumping and I started sleeping better at night.

Enjoy the new sensations. Your body is adjusting. Now you have this wonderful piece of machinery that won't quit, go to sleep, or take a day off. It will quietly sit there and make sure your heart will do what it's supposed to do.

Enjoy life. You've got a lot of living to do.

Is that what you mean? It would mostly happen to me at the gym when working out and I could see it hopping under my shirt when looking in the mirror. Some testing and adjustments took care of it.
