Hi Friends
I am from India, i have PM implanted 4 months back, Medtronic. just before my implantation my echo- LVEF reads 49% and now it decreases to 30%, it is written 60-65% as normal. my cardiologist didn't gave a clear explanation, for this, it seems my endurance level is much below than i was say 2/3 months back. my setting is 60-130 beats. Please explain my decrease from 49 to 30 is because of PM implantation or is this nothing to do with
PM implantation and my heart condition has deterioriated.

You need to have a chat with your dr, that is a large and sudden drop. Why did you get the pm? Sometimes the pm fixes the problem, other times the heart is diseased and all the pm does is try to manage the problem. If you have a simple electrical problem and your heart is structurally healthy, my first thought is inappropriate ventricular pacing or pacemaker syndrome. If you search past posts you should be able to learn more.

Hi TraceyE and PacerRep, please have a look at the full information. I was diagnosed with complete heart block in march this year and implanted Dual chamber rate responsive pacemaker (DDDR) RELIA REDR01 in April. My heart beat before the implantation was 45 to 50 only, so slow heart beat, i could not climb stairs and cannot jog for more than 100 metres. Pacing in the ventricles now is AS-VP 50.9%, AP-VP 48.0%, APVS 1.0%. The LVEF still reads 30% after 3 months of medication of Carvedilol 3.125mg and Ramipril 2.5 mg, have no effect.

yeah so your basically 100% paced in the ventricle which creates a pacemaker Left Bundle Branch Block. This can easily attribute to a decreased LVEF. Since you have CHB there is nothing they can do to decrease your RV pacing. Only thing left is keep playing with the meds or get a device with a 3rd lead to BiV pace.

How far i will go with this 4 month old double chamber, and when i will be ready for BiV pacing?

You will be fine with the Dual chamber for a long time, you may never need the the BiV, and if you do...there is no way for me to predict that from behind my computer 1000's of miles away. Just base it all off of how you feel. If you feel good, then don't worry about it. Don't get fixated on the numbers.

How much are you paced in the bottom?

What is the percentage of pacing in your ventricle? Do you know if you had a "normal" heart beat before the pacemaker (meaning it the electrical conduction system is in tact, it may have been slow but no bundle branch blocks, or AV blocks).
Any medication changes?
Your LVEF can go down if you have normal electrical conduction and then you are paced in the bottom part of your heart.
If appropriate for you....ask them to set your device where you only pace in the top part of your heart...again if that is an appropriate setting for your condition.
