Pain in area of pacemaker

A dear friend of mine had surgery back November 2009 for implanting a pacemaker. Out of nowhere today , he is experiencing severe pain when he bends over or moves upper half of body! Also, when he lays down, he has pain in his neck on opposite side of pacemaker? We are calling the Dr tomorrow, but was wondering if I should take him to the ER today?? Has anyone had this problem? Is this serious, should we go to hospital! Thank you...Lisa
