I was told not because of my pacemaker. But because of my reverse shoulder replacement.

Plenty of opportunity for pathogens to get into your circulation from something as invasive as bridge work.
Simple scale, polish, probably not.
UK NICE changed their guidance a while back (against routine usage of antibiotics for heart implant patients undergoing dental procedures) and there's published evidence that this is wrong.
My experience: dental extraction (upper molar) last year, had symptoms of bacteraemia within 3hrs. Happily on correct antibiotic prophylaxis.
Don't gamble your life: if they recommend prophylaxis, take it.

Bernardino, you should be guided by your dentist and by your cardiologist who will know your medical condition(s).
Before root canal treatment for example, I have had to take antibiotics to help control any infection and/or to reduce swelling before dental work could safely and effectively be done, so it very much depends on why they are recommending the 2 grams of antibiotic? Have you asked?
I know guidelines have changed in many countries about the use of antibiotics prior to and after dental treatment, particularly if we do not have a strong indication for an antibiotic, but only your dentist/cardiologist can really advise on what will be best for you personally?
