EP gave me the all clear at 5 weeks post op. I made it out by 6 weeks. No restrictions back to normal play for all sports.

I was practising my short game from about 4 weeks and nervously played a few 9 hole rounds with a half-swing until 8 weeks when I felt ready to play a normal round. This was with the agreement of my E.P. who is a keen golfer himself BUT E.P.s seem to vary enormously in how conservative they are.
8 weeks felt right for me but I guess we are all different.

I live in an area that's huge for golf so my ep has instructions just for golfers. Putt, chip, pitch whenever you feel up to it. Full swing, 3 months.
