It sounds like you are very concerned about this pain - get it checked out.
There are numerous possible causes so speculation by members here is fruitless

Had a relative that always experienced this symptom as angina. I would contact cardiologist if you haven't already. Any unusual or new symptom needs exploring.

As Angry Sparrow has pointed out, there may be other warning symptoms of pending cardiac event. I've had another family member with left arm pain that resulted in bypass surgery (early 1970's before stents available). 25 years ago, I remember noticing the entire outside of my left leg below the knee was numb and there was an unusual neck pain. Not having experienced either before I thought nothing of it. Not long after, I developed a slight lung pain that i thought was caused by a respiratory infection. This led to bypass surgery. During recovery, I noticed the neck pain and leg numbness were both gone.

The pacer systems are really very reliable. The main problem is the incompetent programming of them. If yours is working well for you, get on with life and enjoy it. You probably are more at risk of problems with a valve job than the pacer.

should be from something else not pacemaker.
