MDIC patient forum for Micra AV on 9/12/2022

Hi I will be speaking about my patient experience at the MDIC for Micra AV. I'll discuss some of the results of my post a while back: https://www.pacemakerclub.com/message/41229/micra-av-self-programming

Good to hear from you again

Erichlin, hope you are having a better experience with your Micra AV and that you can give members news of some valuable tried and tested results.

Can you give us more information about the MDIC (Medical Device Innovation Consortium) patient forum for Micra AV on September 12th, in particular, could you kindly set out briefly your programme of topics you intend covering, so that members with specific difficulties with their Micra AV may comment if they wish?  That would be helpful.

erichlin - Quick question

by Good Dog - 2022-08-25 21:19:33

I have a quick question if you don't mind. You may have mentioned in the old transcript (I read today for the first time) what your most recent EF was and your escape rhythm in bpm. However, I just don't have the time now to re-read it in search of it. So if you don't mind, can you provide that to me? I am just curious, because you sparked my interest in the Micra RV and its impact on your CHB. I am pretty low on the learning curve, but the more I learn, the more fascinated I become. So I am interested.

I am conventionally paced for over 35 years and most recently diagnosed with PM induced HF. I was diagnosed at age 27 with a first degree CHB that progressed to complete block a week after turning 38. I am interested in learning a little more about the syncrony provided by the Micra.

Thank you in advance!

Sincerely,

Dave

Thanks Gemita! I do not yet have planned what I will be discussing, but when I do, I'll report it here.

Good Dog: My EF was around 50 a year ago and my escape rhythm varies from around likely under 30 bpm to around 45 bpm.

The synchrony provided by the Micra as currently set by pacer clinics is not something I would recommend for AV synchrony. However, with the way I have programmed it, it can somewhat work. I'm excited for the Abbot dual chamber leadless in clinical trials.

Thank You!

I appreciate the info! Thanks! Wow, your escape rhythm is much higher than I would have guessed. I suppose that is related to the fact that you are relatively young. When I was in complete block (over 35 years ago), my rhythm fluccuated from 35 down to 12 bpm. They let me lay in the ICU all night and until they implanted a PM the next day. They could have slapped a temp PM on me, but they did not. I was conscious even at 12 bpm. The ICU nurse could not believe it. Her eyes were as big as saucers when she shot in the door of my room. I asked her what was wrong? She simply said that she did not expect to find me conscious. Of course I was lying down and was not moving. Currently, the clinic has not been able to find my escape rhythm when they drop my rate to 30, so I suppose it is much lower now. During my last generator change the Medtronic Tech had to drop my rate to 30, Maybe even slightly lower, and had to wait for a long while for it to pickup. I did not have the opportunity to discuss it with him as I would have liked, because when I came out of the anesthesia, he and my doc were gone. It has been discouraging getting information from my previous EP's. I have a new one at the Cleveland Clinic, so we'll see how that goes?

Thanks again!

Sincerely,

Dave

escape beat rhythm varies similar to sinus

by erichlin - 2022-08-30 19:20:47

what i have noticed empirically with my own heart is that my escape rhythm varies similar to my sinus node. after activity, it can be close to 48 bpm. When I'm about to go to sleep, it drops under my LRL for my pacemaker, so I cannot tell what it is. I just know it is under 30 bpm. I could turn off my pacemaker so to speak to truly find out my escape when my sinus node is at its slowest, but then that is a big risk. So it's possible mine goes much lower than 30 bpm.

I am just grateful to God that I lived long enough to have my ICD put in. So many people are not as lucky as us; even though we sometimes don't feel lucky.
