A lot of things could cause them to say the heart is weak so the first thing to do is find out more specifically what they mean. Some conditions can be improved with exercise, some can be improved with medication, some might benefit from a CRT rather than a standard 2 lead pacer.
Many reasons of weak

as Tracy E stated,, ask the doctors for specifics and describe weak! Weak could be the beat, weak muscles,, weak wall/tissue,, weak ports/valves, weak flowing! My son in law is 38, he has a weak heart, come to find out, he only has 1 valve working! He is beating through 1 atertery and it is herediary, he has lost 2 family members due to this! His heart is deteriorating! He works everyday installing and repairing septic systems and works alone, he doesn't have a lot of pain or problems at this point! I would suggest to your friend to ask about getting on a donors list after the specifics are explained! The sooner the better since he is so young!

Yesterday I moved to a new place in my mind and realized how bad I felt 'before' and the difference my pacemaker has made.
Possible reason

Hearts remodel due to "conventional" pacing. That is why doctors are now implanting the pacing lead to pace the root of the cardiac conduction system (Google that) called the His bundle. See <His-pacing.org> The good news is that patients whom have been paced into heart failure over time find that the heart heals itself after a few months of natural, physiological ventricular activation by the His/Purkinje system. Your heart doctor is well aware of this, and they try to minimize ventricular paced beats.
All the best,
