.........I see that you are from the UK..I'm sure that many will understand though...
So..here goes...
Your Pacemaker basically just sits there...like that bloke in that booth/hut thing at the entrance to a factory...
24 hours a day he (?) sits and watches about half a dozen screens...the car park ..the back lot ..the side alley..etc etc.
Generally, nothing happens..kids in their Astras and other hatches ,..parked side by side...we have all seen it...
BUT..
Sometimes something is suspicious or something kicks off..
That's when PM Man gets onto the case and brings in what is needed to sort the situation...Tra-La !!

What percent of the time?

My pacemaker works 70% of the time; to create the signal to stoke the ventricle. I was worried that it is being used more than I wanted; but isn't it nice to know I have it and that it works! Not sure what the average is for a on-demand pacemaker but I received mine in November of 2013 and I have 4.5 to 6 years left in the battery.

They watch all the time. With av block, they watch for the atria to beat but the ventricle not to follow suit. If the ventricles beat as they should, it continues to watch. If it goes a fraction of a second after the atria with no beat, it sends out the signal to beat.
