Who are you?

You might get more participants if you identified yourself and explained the reason for the survey. How will the data be used and by whom?

Sorry I should have identified, I am Carl Robinson a senior cardiac physiologist, the questionnaire is being 'used' to campaign for better and more thorough information and support to be given to people receiving pacemakers by manufacturers.

P.s. the survey is protected by data protection act and the option is there to not include your name email age or in fact any questions people to not wish to disclose.

He explained who he is and the purpose in the other post. A quick google search was enough for me to think he's legit and do the survey but decide for yourself.
