With any surgical procedure where likely bleeding is involved the Drs would usually stop the Xarelto at least 24 hrs before the op and then re-start it as soon as normal haemostasis ( clotting etc ) is occurring.
In most situations it is no big deal.
BUT I guess it all depends on how big a clotting risk they perceive you to be. I assume that you have some sort of arrhythmia; if this is really severe they would have to weigh up the 2 risks i.e.the risk of clotting from the arrhythmia against the risk of you bleeding excessively from the op. Policies re stopping the Xarelto completely, or not, could occasionally vary from patient to patient.

I had a PM implanted under the muscle on 12/1/14. I stopped taking Xarelto 4 days prior to the procedure and began taking it again 48 hours later. So far, no complications, to include excessive bleeding or hematoma.

I don't know about Xaralto but i'm on Pradoxa and they just had me stop it a couple of days before. very short half life and it's out of your system.
