I've never heard of a home rehab. Part of what makes it different from any other exercise is you are medically supervised and your workout is customized to what you need.

for what its worth

i didn't know it at the time, and no one told me, but when i was rehabbing, i did almost everything myself. that may sound strange but its so tempting to ask others to do things for you, but if you do, it may take you longer to get well. i only asked my wife to help me when i couldn't do something. reaching and stretching for things was painful. climbing stairs (slowly is recommended) yourself rather than asking someone to bring you something. all these efforts YOU make help you rehab faster. walking is the best exercise. when it was too cold i'd go to the mall and walk, and to my surprise there were others there doing what i was doing! its been two years since my pm/df was implanted. i'm feeling well, my doctor says i'm doing great, yet i know my life and abilities have changed forever. i won't ever be as strong or have the endurance that i used to have before the pm, but i also know i'm no longer a "walking timebomb" as i was before.

To tell you the truth I never even give it a second thought. While growing up it never stopped me from doing anything and to this day my girlfriend or my kids need to remind me that I have one!
