This jolting problem Is from one of you leads next to a nerve by the diaphragm. This happens to me all the time and they don't want to move the lead. They say it wont hurt you but it sure does make me jump just as i am falling asleep. Sometimes it is small and other times it about knocks me out of bed and takes my breath away. I have had this for two years and they have tried to lower the settings. So I live with it. As far as the palpitations I still get them off and on, and if they get to bad my crt-d device would correct it or shock me back to normal. Do you have a three lead device or two lead. And some times it takes time for meds. and the device to help you heart get back to as normal as it can. And it depends on the reason you had your pacemaker put in.

u can only hope u necer get shocked

it might be your pacemaker doing a self check that is causing the jolts they shouldn't feel like a jolt so I would talk to the doctor again. If you have issue other than heart related ones,i.e lupus then your body might react differently than what the average person moght feel. Your Doctor should not have been so dismissive of your feeling of impending doom, that is often a precursor to panic attacks which can be controlled by various meds and alternative methods.
best always,

Sorry to hear of your ongoing issues. I am 11 months into my pacemaker, but started out with some of the same issues you have. Jolts especially at night were making it difficult to sleep. On my 3 month device check they played with the settings and over the next 3 weeks the symptoms really improved,so don't be shy about engaging whoever you need to try to improve settings and any issues that should not be accruing. Good luck
