Judging from your description I have to say it is not normal If it were me I would give the doctor a call and get his opinion. If what you described as looking like a vein is very small it could be a lead and the puffiness could indicate infection. You have had your pacemaker a week and while we are all different, at the end of the first week for me mine was still tinder, but otherwise nothing about it got my attention.
You know you're wired when...

I am 100% pacemaker dependant and have been all my life. I try not to think about how a little metal box keeps me alive - it would drive me crazy. So I lead a very active life.
Probably is

Shortly after my surgery over a year ago, the scar looked like a cat scratch. As it healed, the area has risen and there is now a bump running along the cut area.
I suspect that complications are rare. If you have unusual redness and sensivity around the scar, you'll want to have it looked at. Mind you, your operation was less than a week ago, so I expect the area WILL be red and tender.
In any event, "When in doubt, check it out".
